Note cards measure 5” x 7”, are printed on 100# cover and come with 24# A-7 envelopes.

Description of Customization Types

Generic

No customizations. The generic version of the NDLM Note Card has the Donate Life or Done Vida logos and mentions DonateLife.net or DoneVida.org.

Custom – Online Design

Using our design portal, you can make the following customizations to the back: replace “DonateLife.net/DoneVida.org” with your state registry URL; replace the Donate Life or Done Vida logos with a state version or your own logo.

Custom – DLA Design ($100)

For more complex customizations—like adding a message to the inside of the card—select this option. Donate Life America will work with you directly and a $100 customization fee will apply. DLA will contact you once your order is placed to begin the customization process.

Custom – Reorder | Existing Art

Select this option if your organization has ordered a custom 2025 NDLM Notecard from DLA and would like to reprint it with no changes. This does not apply to reprinting last year’s customizations with this year’s artwork.

Custom – Reorder | Updates to Existing Art ($75)

Select this option if your organization has already ordered a custom 2025 NDLM Notecard from DLA and would like to make changes to it before reprinting for a $75 fee.
